Vaccination Record

The COVID-19 Vaccination Record feature in myColorado® is not currently available as we are creating a new experience. In the meantime, you are able to download the records for you or your legal dependents directly from the Colorado Immunization Information System (CIIS), and upload those documents into myCards and Documents within myColorado for easy access.

Access your digital Vaccination Record and add it to myCards and Documents

Tap the COVID-19 Vaccination Record button on the Wallet screen.
Tap on the link to the Colorado Immunization Information System. This will open up in the web browser on your device.
Note: additional instructions from CIIS on how to use the CIIS Public Portal are available here.
Indicate if you are obtaining records for yourself (Me) or a Dependent.
Input your Name, Date of Birth, Gender, and method to receive your access code, and then click Get Access Code.
On the Verify Your Identity screen, Input the code that is emailed or sent by SMS, and click Verify.
Choose from the options to Download Full Record, Download School Certificate, Download COVID-19 Record, or View COVID-19 QR Code, and save the PDF or QR Code to your device.
Return to myColorado, and click on myCards and Documents in the Wallet.
Click on Upload a card or document, or press the “+” in the upper right.
Click Upload PDF, and choose the document you downloaded from CIIS.
Enter a Document Name, such as “Vaccination Record”, and press Submit.

Tap Upload PDF.

Choose the PDF downloaded from the CIIS portal.

Input the name you would like the document to display.

Show proof of vaccination

Tap the myCards and Documents button on the Wallet screen.
Tap on the document name you gave to your Vaccination Record or QR code to view and present the document as needed.

Questions about your Colorado immunization record?

Contact the Colorado Department of Public Health and Environment (CDPHE) at cdphe.ciis@state.co.us. If you are having difficulty accessing your record or are seeing an incorrect record, submit CDPHE's Immunization Record Information Request Form. In some cases, you may need to speak with the provider who administered your vaccine to confirm they submitted your correct information.

COVID-19 Vaccination Record Record FAQs

How do I refresh my vaccination record after receiving additional vaccinations or doses?

You will need to connect to the Colorado Immunization Information System (CIIS) portal and download your updated record with the latest information, and add that record to myCards and Documents.

How soon after receiving a vaccine, including a booster, can I access the vaccine record?

After you receive your vaccine, your healthcare provider should promptly report your vaccination to the CIIS. This time may vary, so please allow 24-72 hours for your vaccination to appear in your vaccination record.

My contact information was recently updated in CIIS. When can I expect to be able to access my record?

It will take a minimum of 24 hours for your updated contact information to reflect on your account.

I was vaccinated in another state, how do I report this to Colorado so I can see it in my Vaccine Record?

If you are a Colorado resident and have a Colorado driver license or state-issued ID, you can call your provider in the state you received your vaccine and ask them to report your vaccination record via email to CIIS at cdphe.ciis@state.co.us. As soon as your vaccination information has been reported to CIIS, you can download your updated record from the CIIS portal.

Can I access my child’s vaccine record?

You are able to access the records for you or your legal dependents through the CIIS portal.

Will my immunization information remain private?

Yes! Use of the myColorado app to store vaccination records is voluntary on an opt-in basis. No data will be accessed or shared except that which is requested by a myColorado user who has been authenticated through facial recognition. A retrieved vaccination record is only visible to the myColorado user who downloaded it, and the data is not shared with anyone else in the retrieval process.

I don’t want my vaccination record card displayed anymore. Can I delete it?

Yes. In myCards and Documents, click the Trash Can icon next to the vaccination record, or any other documents you want to remove, and follow the prompts.

Troubleshooting Tips

I previously linked my vaccination record to the app, and it was not moved to myCards and Documents.

If your record was not accessed in the myColorado app for some time, the link between the app and your record may have been expired, so we could not automatically move it into myCards and Documents. Any documents you choose to upload into myCards and Documents will remain until you choose to delete them.

The record I see in myCards and Documents is outdated.

Documents and images uploaded to myCards and Documents are not automatically refreshed when there is new information. If you information has changed, you will need to download the latest information from CIIS and upload an up to date document into myCards and Documents. Be sure to delete your old documents if they are no longer needed.

Where did my downloaded vaccine record go (Android users)?

Some LG and Motorola phones don’t come with an app that lets you open downloaded PDF documents. If you have downloaded your vaccine record but cannot locate the file on your phone, you may need to install a PDF reader app on your phone to access the file. Several free apps are available on Google Play.

Where did my downloaded vaccine record go (iOS users)?

Download the PDF and access via your Files app. Click Browse -> iCloud Drive -> myColoradoicloudcontainer folder.

If you are not able to locate your PDF, download in your Files app or you receive a “Sign into iCloud” error, please check your iOS settings. First, confirm that you are signed into your iCloud account. Once you are signed in, make sure that your iOS Wallet app is enabled by accessing your phone Settings -> Apple ID -> iCloud -> Wallet. Tap the toggle so that it is green or in the “on” position. To disable your Wallet, tap the slider again until it turns grey or in the “off” position.
